Asthma
ConditionCare gives you 24/7 access to a team of health professionals, including nurses, dietitians, pharmacists and social workers, who can answer your questions, help you manage your symptoms, and provide advice about:

- How to quit smoking
- Pain management
- What your symptoms could mean

Breast health
Your PPO coverage includes women’s health services, such as mammograms, annual gynecological exams, family planning services, and other care. You can also take advantage of the Athena Breast Health Network, which serves hundreds of thousands of women who receive breast care at the six UC academic health centers and their affiliates. Athena helps women evaluate and manage their breast cancer risk, access resources, and develop a personal plan with their health care provider. When you receive a mammogram, you are also eligible to use Athena’s breast health screening tool, which provides additional information about your risk of developing breast cancer. And if you’re found to be at a high risk, you can receive counseling by phone or in person from an Athena breast health specialist.
Cardiovascular, COPD and heart disease
ComplexCare nurse care managers trained in helping higher-risk patients can work with you and your treating doctor to create a personal nursing care plan.

Diabetes prevention
Lark: Offered at no cost to you through Anthem, Lark is a program that offers tools and techniques to help reduce your risk or developing diabetes. Lark coaching includes topics such as sleep and stress reduction. Take the quiz to see if you’re a candidate for the Lark program.

Preventive care
Medicare covers many preventive services that keep you healthy, including an initial preventive physical examination, annual wellness visits, shots, lab tests and screenings. Medicare preventive care also includes programs for health monitoring and counseling and education to help you take care of your own health.

Tobacco cessation
The following highly rated do-it-yourself apps can help you take charge of your habit and quit for good:
- Smoke Free (free): Over 20 different, evidence-based techniques to help you become — and stay — smoke free. Download from Google Play or the App Store.
- Kwit (free): Various game techniques, mechanics and thinking to encourage you to stop smoking. Download from Google Play or the App Store.
- LIVESTRONG MyQuit Coach: A personalized plan to help you finally stop smoking, once and for all, at a pace that works for you. Download from the App Store. (Not available for Android.)
